SPRING KINDERGARTEN/PRE-K REGISTRATION

H. B. SUGG ELEMENTARY SCHOOL WILL BE HOSTING KINDERGARTEN/PRE-K

REGISTRATION ON MONDAY, MARCH 2, 2009 FROM 8:30 AM-1:00 PM, 

TUESDAY, MARCH 3, 2009 FROM 8:30 AM – 1:00 PM AND 5:00 PM-7:00 PM WEDNESDAY, MARCH 4, 2009 FROM 8:30 AM-1:00 PM.

 

  YOUR CHILD MUST BE 5 YEARS OLD ON OR BEFORE AUGUST 31, 2009 TO REGISTER FOR KINDERGARTEN.  YOUR CHILD MUST BE 4 YEARS OLD ON OR  BEFORE AUGUST 31, 2009 TO APPLY FOR PRE-K SCREENING .  REGISTRATION WILL BE HELD ON OUR CAMPUS AT 3992 GRIMMERSBURG STREET.

We have provided a checklist below of documentation that you must provide when registering your child for Kindergarten.  Please understand that your child cannot be enrolled without all of the required information.

 

__Two (2) proofs of residence that reflect the physical address at which you and your child reside (items must be addressed to you).

Acceptable Documentation Includes:

Current Voter Registration

Pay Stub

Utility Bill

Receipt for property tax

Contract for sale or lease of home

Current Automobile Insurance Policy

Financial Statement from NC regulated Financial Institution

Document issued by the State of North Carolina

Document issued by an Agency of the United States

 

__Completion of a Pitt County Schools Registration Form (provided by school)

 

     __Certified Birth Certificate

 

     __Current Immunization Record

 

__Kindergarten Health Assessment Report (completed by your child’s physician after a physical exam) – The Health Assessment must be current and can not

  be accepted if completed more than 12 months prior to the first day of school.

 

     __Home Language Survey (provided by school)
